Here is a shot in the dark. I saw this software demonstrated and I was amazed at what the (trained demonstrator) could do with it, but I have no real experience with it and am not sure if it would actually work for you. You may have heard of it its Dragon Naturally Speaking voice recognition software. You could even use their headset to listen to the audio book.

Maybe I didn't make this clear; my main intended use would be with my 600E, lid closed in a backpack: I'm listening to an audiobook on headphones, and somebody on the bus would like to know where "Main Street" is. I would like a way to pause the playback, or even to go backwards if I start daydreaming. I don't want to have to tell the person to fluff off because I have no control over playback. I want to have at least a subset of the control I could with an MP3 portable. So the solution cannot be bulky or require external power. It should be the size of a mouse or remote control.
